Case 1: AC Joint Arthritis

History/Exam

A 42-year-old man presented to the orthopedic clinic reporting 18 months of right shoulder pain. He first noticed the pain after doing push-ups and had off and on pain with increasing frequency since, mostly after strenuous activity (re-aggravated cleaning up after a flood) or attempted throwing. He had previously been seen at an orthopedic clinic closer to his home, where he had undergone multiple injections. A subacromial injection did not seem to help, but two separate injections into the AC joint both relieved his symptoms almost completely for 1 week’s duration.

On physical examination, the patient had full and painless shoulder range of motion, symmetric to his contralateral shoulder. He had a positive O’Brien’s active compression test, with pain localized to his AC joint. He had focal tenderness to palpation over his AC joint. His Speed’s and Yergason’s tests were negative. He did not have any pain with palpation of his bicipital groove. He was neurovascularly intact in this extremity and without symptoms in his contralateral shoulder.
